New intramedullary locking nail for olecranon fracture fixation--an in vitro biomechanical comparison with tension band wiring.

The aim of this study was to determine the difference in displacement of a newly designed intramedullary olecranon fracture fixation device compared with multifilament tension band wiring after 4 cycles and 300 cycles of dynamic continuous loading.In eight pairs of fresh-frozen cadaver ulnae, oblique olecranon fractures were created and stabilized using either newly designed intramedullary olecranon nail or multifilament tension band wiring. The specimens were then subjected to continuous dynamic loading (from 25 N to 200 N) using matched pairs of cadaveric upper extremities. INTRODUCTION The aim of the present study was to develop a test setup with continuous angle alteration to imitate elbow joint motion for the mechanical evaluation of tension band wiring and a newly designed intramedullary nail. MATERIALS AND METHODS The servo-pneumatical test stand worked with a rotational angle-adjusted and a linear force-adjusted engine. Summary Purpose of review This article reviews current best practice for the diagnosis and treatment of comminuted patellar fractures. Recent findings Patellar fractures make up about 1% of all fractures. As a rule, fractures of the patella are caused by direct trauma to the knee. A transverse fracture is the most common fracture type. Open reduction and internal fixation is the treatment of choice for the majority of displaced patellar fractures. Treatment must achieve anatomic reduction of the articular surface and reestablish the continuity of the extensor mechanism. State-of-the-art stabilization techniques are tension band wiring (TBW) using Kirschner wires or cannulated screws. Functional results after operative treatment of olecranon fractures.

In a retrospective, nonrandomized study, we reviewed our experience with the operative treatment of olecranon fractures to find out which factors correlate with subjective complaints and functional outcome. A total of 95 consecutive olecranon fractures were operatively treated in our level I trauma institution between March 1, 1992 and March 1, 2000. Medical charts and radiographs of all patients were evaluated. Three patients died, 23 were lost to follow-up. Fifty-eight of 69 available patients (84.0%) were reviewed clinically and/or radiologically after a mean time interval of 36 months.
